Job summary

Transitions Hospice - Illinois is seeking a compassionate Companion Aide CNA to provide overnight bedside presence in Elgin and surrounding northern Illinois. The 8pm–8am PRN shift emphasizes safety, personal care, and family reassurance with strict confidentiality.

You will monitor patient condition, document changes, assist with repositioning, hygiene, and mobility, and report to the on-call nurse. The role offers competitive hourly pay and company-provided equipment, plus PTO.

Qualifications

• Current CNA certification meeting state training requirements.
• Current CPR certification.
• 1+ year of hospice or related healthcare experience preferred.
• Overnight or night shift experience preferred.
• Ability to work independently and stay alert overnight.
• Strong observational skills to report changes in condition.
• Genuine compassion for end-of-life care.

Responsibilities

• Provide continuous bedside presence and patient supervision overnight.
• Monitor patient condition and environment; report changes to the RN.
• Prevent falls or self-harm within CNA scope of practice.
• Assist with personal care and mobility as assigned.
• Observe and document patient condition and vital signs when ordered.
• Support families with overnight respite and communication.
