Job summary

Transitions Hospice is seeking a Night Nurse to join the interdisciplinary care team for overnight coverage in Bloomington, IL. You will assess, treat, and communicate patient needs with clinical confidence and compassion, delivering care across the full nursing process.

Requirements include an active RN license in the state, nursing degree, and the ability to work independently at night. Hospice/home health experience is preferred, with strong documentation and communication skills driving

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Comfort and composure working independently during overnight hours.
• Strong assessment skills and sound clinical judgment in high-pressure moments.
• Excellent documentation habits and communication skills.

Responsibilities

• Deliver professional nursing care across the full nursing process during overnight shifts.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet overnight patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond with clinical judgment.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support continuity of care.
• Ensure equipment and necessary services are in place for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.
