Job summary

Qualifications

• High School graduate required.
• Experience in direct patient care or patient observations preferred.
• One year of progressive education in a human services-related field acceptable.

Responsibilities

• Conduct continuous or intermittent patient observation as assigned.
• Report changes in patient condition to nursing or clinical staff promptly.
• Monitor and document patient behavior, mood, and activity per established protocols.
• Assist patients with daily activities such as hygiene, meals, and mobility as needed.
• De-escalate situations using approved verbal intervention techniques to promote safety.
• Maintain care areas free of hazards and ensure clean, orderly spaces.
