Job summary

Responsibilities include observing patients, reporting changes, assisting with daily activities, de-escalation when needed, and ensuring safety.

Qualifications

• High School diploma or equivalent required.
• Experience in direct patient care or observations preferred.
• Progressive education in a human services-related field is advantageous.

Responsibilities

• Observe patients continuously or intermittently as assigned.
• Report changes in patient condition promptly to nursing/clinical staff.
• Document patient behavior, mood, and activity per protocols.
• Assist with daily activities (hygiene, meals, mobility) as directed.
• De-escalate situations using approved verbal techniques to ensure safety.
• Attend staff development, supervision, and meetings as assigned.
• Maintain safety, cleanliness, and confidentiality in patient areas.
