Qualifications

• Current RN license to practice in the state of Utah.

Responsibilities

• Assess patient's physical, social, and psychological status and gather information from the care team.
• Develop and implement an individualized plan of care with patient and family involvement.
• Coordinate and delegate care with interdisciplinary team to meet goals.
• Evaluate patient responses and revise plans as indicated.
• Maintain accurate documentation and comply with licensure standards.
