Qualifications

• BSN is required as of 4/27/2025 per job description update.

Responsibilities

• Assess patient physical, social, and psychological status and gather information from the interprofessional team.
• Collaboratively plan and educate to prepare patients for follow-up needs.
• Direct the care team to implement the plan and manage time, supplies, and resources.
• Evaluate patient responses and revise the care plan as needed.
