Qualifications

• BSN and Illinois Registered Nurse license are required.

Responsibilities

• Collects and analyzes data, identifies outcomes, develops a plan and evaluates progress.
• Directs care and utilizes any assistant involved in direct patient care.
• Collaborates with patient, family, physicians and staff to document an outcomes-focused care plan.
• Participates in governance, committees, and unit projects.
• Documents patient responses and tailors plans based on evaluation.
• Uses evidence-based practice to guide nursing decisions.
