Job summary

The Gardens at Collinwood in Fort Collins, CO is seeking a Full-Time Night Shift QMAP to join our compassionate care team. The shift runs from 10:00 PM to 6:30 AM and includes some weekends and holidays.

Qualifications

• Interpersonal skills to communicate with residents, staff and families.

Responsibilities

• Sets up medications and passes them to residents following established policies and procedures.
• Performs narcotic counts at the beginning and end of each shift.
• Orders medications from pharmacy following community policies.
• Checks MARs for errors following community policy.
• Calls residents' families to bring in medication as needed if applicable.
• Administers treatments, takes vital signs, and weighs residents.
• Documents changes and orders on MAR per Dr. orders.
• Schedules doctor appointments as needed.
• Assists residents with showers, grooming, dressing and toileting as needed.
• Assists with activities of daily living as described on care plans.
• Maintains confidentiality and positive communication with residents and families.
