Job summary

Family Health West in Fruita, CO is seeking a Resident Care Specialist with QMAP for night shifts. This on-site role involves delivering compassionate personal care, assisting with activities of daily living, and documenting care in EMR. Applicants must obtain QMAP within 6 months and be able to work weekends and holidays.

Wages start at $20.00/hour with night/weekend differential. Immunizations including flu vaccine are required.

Qualifications

• Must be compassionate and caring to work with residents.
• Must be able to communicate effectively and professionally, both verbally and in writing.
• Must be willing to obtain and maintain QMAP certification within six months of hire.
• Must be able to work weekends and holidays.

Responsibilities

• Promotes resident independence in accordance with service plan.
• Assist with daily personal care tasks including bathing and grooming.
• Maintain a clean and safe environment and follow house rules.
• Provide documentation in EMR accurately and timely.
• Assist with transfers and mobility aids as needed.
• Respond to safety or medical issues and perform minor first aid.
