Job summary

The Estate at Hyde Park is seeking a Licensed Practical Nurse (LPN) to provide licensed nursing services in memory care. The role covers 7p-7a shifts with alternating weekends, full-time status, and requires a Florida LPN license and CPR certification.

Responsibilities include medication administration, assessments, care planning, and patient/family communication, with collaboration across departments. HP safety plans and confidentiality are essential. A robust benefits package is provided.

Qualifications

• Must have a Licensed Practical Nurse license current in the state
• CPR certification required
• 1+ years in Senior care or related field

Responsibilities

• Medication administration for residents per regulations and company guidelines
• Assist with admissions and assessments
• Develop and document care plans
• Monitor health changes and communicate with physicians and families
• Maintain confidentiality of resident information
• Provide education to residents and staff
• Respond to emergencies and coordinate with the safety plan
