The Estate at Hyde Park in Tampa, FL is seeking a Licensed Practical Nurse (LPN) to provide licensed nursing services in memory care on a part-time/PRN basis. You will administer medications, conduct assessments, and contribute to care planning under Senior Lifestyle Corporation guidelines.

The role requires 1+ year of senior care experience, an active LPN license in the state, CPR certification, and English proficiency.

Qualifications

• Must have a Licensed Practical Nurse license current in the state in which they are practicing.
• CPR certification required.
• 1+ year experience in senior care or related field.
• English proficiency to perform duties and communicate with supervisor and staff.
• Basic computer skills are expected.

Responsibilities

• Administer medications in compliance with Federal & State regulations and guidelines.
• Assist with admissions including assessments and documentation.
• Develop and update acute and long-term care plans or service plans.
• Track clinical information (e.g., wounds, infection control) as directed.
• Provide health-related consultations within the community.
• Notify physicians/families of health changes with proper documentation.
• Respond quickly to emergencies and maintain communication with departments.

Do you have HEART? This position is responsible for providing licensed nursing services to residents, following the guidelines provided by Senior Lifestyle Corporation.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand roll in the Safety & Disaster Plan.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
