Job summary

Colfax Health and Rehabilitation of Cascadia is seeking a compassionate Licensed Practical Nurse (LPN) for the nocturnal shift. You will deliver direct nursing care, monitor residents, and coordinate with RNs and physicians to ensure safe, high-quality care throughout the night.

Under RN supervision, you will oversee medication administration, support care planning, and document interventions. Strong assessment, quick decision-making, and excellent communication are essential for resident

Qualifications

• Current LPN license in good standing.
• Current CPR certification or ability to obtain upon hire.
• Strong clinical assessment and documentation skills.
• Ability to respond quickly and confidently in emergency situations.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct nursing care consistent with scope of practice and competency.
• Monitor resident conditions and communicate changes using SBAR.
• Collaborate with RNs, physicians, consultants, and interdisciplinary team members.
• Participate in resident assessments, care planning, and care plan updates.
• Administer medications and treatments safely and accurately.
• Maintain narcotic and medication records and coordinate orders.
