Job summary

The Cascades Health and Rehabilitation Center is seeking a compassionate Licensed Practical Nurse (LPN) to provide direct patient care under supervision. Responsibilities include administering medications, taking vitals, wound care, and assisting patients with daily living activities within a team of healthcare professionals.

Qualifications include a current LPN license, graduate from an accredited practical nursing program, and experience with EMRs preferred.

Qualifications

Current state license as a Licensed Practical Nurse.
Graduate of an accredited practical nursing program.
Knowledge of nursing principles and practices.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
Attention to detail and multitasking ability.
Flexibility to work varied shifts, including weekends and holidays.
Experience with electronic medical records (EMRs) is preferred.

Responsibilities

Administer medications as prescribed by a physician or registered nurse.
Monitor and record vital signs.
Communicate with patients and families about care and treatment plans.
Document patient visits and treatment plans in EMR.
Collaborate with healthcare professionals including RNs, physicians, and therapists.
Complete patient assessments and coordination of care.
Identify and report changes in resident condition to RN.
Follow infection control and safety protocols.
Maintain confidentiality and ethical standards.
