Qualifications

• LPN degree required.
• Ability to creatively problem-solve in resident care and staff management.
• Minimum of 2 years in geriatric nursing, including med administration and pharmacy coordination.
• Basic computer knowledge; ability to learn company applications.

Responsibilities

• Foster resident-centered care planning and dignity for residents.
• Assess residents’ health and psychosocial status; develop individualized service plans.
• Coordinate with physicians, home health, labs, and pharmacies to meet care needs.
• Delegate nursing tasks per laws and communicate with residents and families.
• Document all resident care promptly and maintain records per policies.
