Job summary

Colfax Health and Rehabilitation of Cascadia is seeking a compassionate Licensed Practical Nurse (LPN) for the nocturnal shift. You will deliver direct nursing care, monitor residents, and coordinate with RNs and physicians to ensure safe, high-quality care throughout the night.

Under RN supervision, you will oversee medication administration, support care planning, and document interventions. Strong assessment, quick decision-making, and excellent communication are essential for resident

Qualifications

• Current LPN license in good standing.
• Current CPR certification or ability to obtain upon hire.
• Strong clinical assessment and documentation skills.
• Ability to respond quickly and confidently in emergency situations.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide direct nursing care consistent with scope of practice and competency.

Rotating Schedule of 6 on 7 off.

As a NOC Shift LPN, you'll play a vital role in ensuring our residents receive safe, high-quality care throughout the night. Working under the direction of a Registered Nurse (RN), you will provide skilled nursing services, oversee medication administration, coordinate care, and serve as an advocate for residents and their families. Your clinical expertise and commitment to excellence help create a comfortable, supportive environment where residents can thrive.

What You'll Do

Provide Exceptional Resident Care

• Deliver direct nursing care consistent with your scope of practice and competency.
• Monitor resident conditions and communicate changes using the SBAR process.
• Collaborate with RNs, physicians, consultants, and interdisciplinary team members to support quality outcomes.
• Participate in resident assessments, care planning, and care plan updates.

Lead Medication & Treatment Management

• Administer medications and treatments safely and accurately.
• Monitor residents for medication effectiveness and adverse reactions.
• Maintain accurate narcotic and medication records.
• Coordinate medication orders, refills, and pharmacy communications.
• Ensure medication carts and treatment supplies remain organized and fully stocked.

Support Clinical Excellence

• Review, transcribe, implement, and communicate physician orders.
• Complete admission and discharge documentation.
• Assist with consultant visits and follow through on clinical recommendations.
• Document resident care, observations, and clinical interventions in accordance with company policies and regulatory requirements.
• Provide emergency response support, including CPR when needed.

Be a Trusted Team Partner

• Supervise and support Medication Aides/Technicians.
• Communicate effectively with residents, families, physicians, and team members.
• Help promote a culture of safety, respect, and person-centered care.
• Maintain dependable attendance and punctuality for scheduled shifts.

What We're Looking For

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification or ability to obtain upon hire.

Requirements

Education

Graduate of accredited LPN School of Nursing.

Licenses/Certification

Current valid LPN license in the state employed.

Valid CPR certification.

Experience

Six months experience in a long-term care environment preferred.
