Pioneer Human Services is hiring a Health Coordinator (Fulltime Nights) to coordinate and monitor client health services in residential treatment facilities. You will liaise with clients, nursing staff, medical providers, and program staff to ensure continuity of care.

The role requires organizing care, documenting health information, and supporting clients with complex medical and behavioral health needs within a structured setting.

Qualifications

• High School Diploma or Equivalent.
• 1 year of caregiving or human services experience.
• Must apply for Agency Affiliated Counselor registration within 30 days of hire and maintain active status.

Responsibilities

• Coordinate and monitor client physical and behavioral health services within residential facilities.
• Assist with intake, telehealth appointments, and medication documentation in eMAR/MAR.
• Provide direct client support in a structured residential environment and support care plans.
