Job summary

Oak Hill is hiring a Direct Support Professional in Bloomfield, CT. The role supports individuals with daily living, health and wellness, community inclusion, nutrition, mobility, and documentation in an agency setting.

Requires 18+, HS diploma or GED, valid driver's license, and ability to pass background checks and trainings. Union non-exempt, overtime-eligible with 25 hours/week and Friday-Sunday night shifts. compensation starts at $20.39/hour with differential pay options.

Qualifications

• Must be at least 18 years of age.
• High school diploma or GED.
• Valid driver's license with the ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including occasional shifts beyond schedule.

Responsibilities

• Supporting Daily Living Activities: assist with personal care, continence care, independence.
• Supporting Health and Wellness: monitor health information and medications per plan.
• Promoting Community Inclusion: transport to appointments and activities; encourage participation.
• Assisting with Nutrition and Mealtime Support: prepare meals per menus and preferences.
• Providing Physical Support: assist with mobility and transfers using approved methods.
• Maintaining Documentation and Compliance: document in EHR and follow regulations.
• Responding to Emergencies and Behavioral Needs: follow procedures; use First Aid/CPR if needed.
• Working as a Team: communicate with staff, individuals served, and families; attend trainings.
