Job summary

Four Corners Community Behavioral Health, Inc. seeks a Case Manager/Certified Peer Support Specialist for a full-time graveyard shift (6 pm–6 am) at the Crisis Receiving Center in Price, UT.

You will greet clients, assist with intake, and help ensure safety and adherence to center rules. You will perform crisis de-escalation, connect clients to community resources, coordinate care, and participate in discharge planning while maintaining a clean, secure environment.

Qualifications

• Must hold a Certified Peer Support Specialist credential.
• Experience in crisis settings preferred.
• Ability to lift up to 20 lbs and stand/sit for periods.
• Valid driver’s license and background check required.

Responsibilities

• Greet clients in a crisis setting and assess immediate needs.
• Assist with intake paperwork and explain center rules.
• Provide crisis de-escalation and connect clients to resources.
• Maintain a clean, safe work environment and daily center tasks.
• Assist with discharge planning and communicate with staff.
• Lift up to 20 lbs and may transport clients as needed.
• Work full-time, graveyard shift (6 pm–6 am).
