Four Corners Community Behavioral Health, Inc. seeks a Case Manager/Certified Peer Support Specialist for a full-time graveyard shift (6 pm–6 am) at the Crisis Receiving Center in Price, UT.

You will greet clients, assist with intake, and help ensure safety and adherence to center rules. You will perform crisis de-escalation, connect clients to community resources, coordinate care, and participate in discharge planning while maintaining a clean, secure environment.

Qualifications

• Must hold a Certified Peer Support Specialist credential.
• Experience in crisis settings preferred.
• Ability to lift up to 20 lbs and stand/sit for periods.
• Valid driver’s license and background check required.

Responsibilities

Four Corners Community Behavioral Health is seeking a Case Manager/Certified Peer Support Specialist. Full-time, Graveyard shift, 6 pm to 6 am. Duties include greeting clients in a crisis setting and assisting in supporting client’s needs throughout their stay at the Crisis Receiving Center. Assisting clients with intake paperwork, notifying clients of the rules of the center, and ensuring a positive and safe work environment are critical. Assisting with crisis de-escalation by assessing client’s needs and connecting them to resources within the community are essential functions of the job. Maintaining a clean and safe work environment is also an essential job duty, and completing day-to-day cleaning tasks for the center will be required. Must be able to lift 20 lbs. and bend, sit, and stand for periods of time. Communicating discharge plans with clients and staff, as well and day to day communication with staff is also required. Some transportation of clients may be required. Candidates must also work with staff to ensure individuals receive the utmost care and compassion throughout their experience. Strong coordination skills and relevant work experience preferred. 40 hours/week with an excellent benefits package. Starting pay is $18.57/hr; salary depends on experience and education. We offer a $2.00/hour differential for shifts worked on Saturdays and Sundays and between the hours of 11 p.m. to 5 am. Up to an additional $4.00/hr. available!
