Job summary

Ohio’s Hospice of Central Ohio is seeking an LPN – Crisis Care to provide one-on-one bedside care for patients who are actively declining, primarily during night hours in Columbus, OH.

Under the supervision of an RN, you will monitor patient condition, document care, and collaborate with families and the care team to deliver compassionate end-of-life support. Nights, 8:00 PM–8:00 AM, three 12-hour shifts with every-other-weekend rotation.

Qualifications

• LPN license in Ohio with no active board actions.
• One year of nursing experience preferred.
• Current CPR certification.
• Computer skills to document patient care accurately.
• Valid driver's license with safe driving record.
• Ability to pass background check and 10-panel drug screen.

Responsibilities

• Provide high-quality nursing care to hospice patients in homes and facilities.
• Provide one-on-one bedside care for patients experiencing an active decline.
• Monitor patient condition and communicate changes to RN and care team.
• Deliver comfort-focused end-of-life care with dignity and respect.
• Document care accurately and timely.
• Collaborate with patients, families, and interdisciplinary team.
