Job summary

UofL Health in Louisville, KY seeks a Clinical Pharmacist to deliver comprehensive pharmaceutical care for patients across neonatal to geriatric ages. You will participate in multidisciplinary rounds, evaluate medications for safety and efficacy, and support drug information needs and pharmacokinetic dosing.

The ideal candidate has a PharmD from an ACPE-accredited school, completion of a PGY-1 or 3+ years of experience, Kentucky licensure or eligibility, and board certification or eligibility

Qualifications

• Pharmacist must be ACPE-accredited, graduate of an accredited College of Pharmacy.
• Post-Graduate Year 1 Pharmacy Residency or 3 years of relevant work experience required.
• Licensure as a pharmacist in Kentucky or eligible to obtain licensure.
• Board certification by Board of Pharmacy Specialties (or within 2 years from date of hire) and BLS/ACLS certifications as required.

Responsibilities

• Medication order evaluation for appropriateness and effectiveness; verification.
• Drug distribution support and technician supervision.
• Clinical intervention and formulary management.
• Provide pharmacokinetic dosing and drug information requests.
• Participate in multidisciplinary rounds and code team responses.
• Document interventions in medical record and cost avoidance.
