Job summary

Prairie Community Services is seeking a Direct Support Provider for a full-time awake night position at Longwood Home, Long Prairie, MN. You will supervise and assist individuals with daily living activities, meals, medication management, and community orientation in an on-site setting.

Starting wage is $16.75–$18.75 per hour. DHS background clearance, a valid driver’s license, and English literacy are required; CPR/First Aid may be administered as needed and daily charts maintained.

Qualifications

• Must be 17+ years old and able to pass DHS background clearance.
• Must have a valid driver’s license.
• Ability to read, write, and understand English in daily work.
• Follow each person served’s program plans and procedures.
• Provide person-centered support and CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety in daily living, recreation, community orientation, medication management, meal preparation, laundry and cleaning.
• Support individuals in line with person-centered plans and procedures.
• Maintain accurate daily charts and documentation of program activities.
