Job summary

Prairie Community Services in Warroad, MN is seeking a Direct Support Provider to supervise, care for and support individuals in daily living, recreation and community activities. This full-time, on-site role includes tasks such as medication management, meal prep, laundry and incident reporting, with a starting wage of $16.75–$18.75 per hour.

You will follow person-centered plans, document daily progress, and maintain safety while promoting independence.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Support daily living activities, recreation and community orientation.
• Administer medication management as needed.
• Assist with meal preparation and laundry/cleaning tasks.
• Document daily program information and progress.
