Qualifications

Responsibilities

• Education - identifies learning needs and uses patient education materials relevant to patient population.
• Evaluation of care - participates in evaluating patient outcomes with the care team.
• Implementation - delivers care for patients with complex problems following standards.
• Leadership - supports unit goals and promotes collaboration with staff.
• Patient assessment - determines priorities and safety needs in plan of care.
• Planning - collaborates with family to update the care plan using multidisciplinary resources.
