Vanderbilt Health in Nashville, TN, is seeking a dedicated RN to join the MCJCHV Case Management team. The role collaborates with the health care team to coordinate patient and family interventions across the continuum of care, removing barriers and promoting efficient use of resources.

The Neonatal Intensive Care Unit will be a primary focus. Shift is Mon-Fri 40 hrs/wk, 8:00 AM - 4:30 PM with some weekend coverage; relocation stipend may be available.

Qualifikationen

• Bachelor's degree in Nursing and RN licensure.
• Minimum 3 years of relevant nursing experience.
• Experience in case management or care coordination preferred.

Aufgaben

• Assess patients with complex clinical needs and coordinate a plan of care.
• Develop case management plans to attain patient and family outcomes.
• Analyze progress and remove barriers using process improvement methods.
• Incorporate ethical considerations in care delivery.
• Provide leadership and serve as a resource to the interdisciplinary team.
