Job summary

Akron Children's Hospital – Akron Main NICU is seeking a Part Time Case Manager to oversee hospitalization of a caseload from pre-admission through one week post-discharge. This onsite role requires a BSN and RN licensure, with 3–5 years of relevant experience.

Proficiency in MS Office and Epic EMR is required, and CCM/CCCTM certification is preferred. Work week totals 24 hours across 8a–4:30p shifts with a 0.6 FTE. Health and safety standards apply.

Qualifications

• Bachelor's degree in Nursing (BSN) is required; Master's degree preferred.
• RN licensure is required.
• Current AHA BLS certification is required.
• 3–5 years of relevant experience; supervisory experience not required.
• Experience in case management preferred.
• Experience working with all levels within an organization.
• Proficiency in MS Office and EMR software (Epic) required.

Responsibilities

• Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge.
• Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.
• Completes educational responsibilities.
• Performs leadership duties.
• Performs quality assurance and performance improvement duties.
• Performs communication and networking duties.
• Performs systems based practice duties.
• Demonstrates the knowledge and skills necessary to provide care for the physical, psych/social, educational, and safety needs of the patients served regardless of age.
