Job summary

Akron Children's Hospital is seeking a part-time Case Manager to oversee the hospitalization and discharge planning for a pediatric caseload in the NICU. You will coordinate care with the interdisciplinary team to meet discharge goals and link families with community resources.

Qualifications include a BSN, Ohio RN license, and 3–5 years of healthcare/case management experience. Proficiency in MS Office and EMR systems is expected. This role is onsite in Akron, OH.

Qualifications

• Nursing degree from an accredited school.
• Licensed and registered as a professional nurse in Ohio.
• BSN required; master's degree preferred.
• Case Management certification preferred.

Responsibilities

• Evaluates and assesses homegoing needs for safe discharge.
• Plans and coordinates care with other care areas and community resources.
• Assesses and procures community resources for changing patient needs.
• Develops materials to facilitate discharge/transition planning.
• Demonstrates leadership and effective communication with care teams and families.
• Provides holistic care addressing physical, psychosocial, educational, and safety needs.
• Performs other duties as required.
