Qualifications

• Graduate from an accredited school of nursing.
• Licensed and currently registered as a professional nurse in the State of Ohio.
• BSN required; master's degree preferred.

Responsibilities

• Evaluates and assesses patients' homegoing needs and coordinates safe transition from hospital to home.
• Plans and coordinates medical care and services with other patient care areas and community resources.
• Assesses and procures community resources for families and refers as needed.
