Job summary

Akron Children's is seeking a Part Time Case Manager to oversee a caseload from pre-admission through one week post-discharge. The role is onsite in Akron Main NICU with a 24 hours/week schedule.

You will coordinate care with the health care team, provide education, and perform leadership and quality improvement tasks while supporting patients across physical, psychosocial, educational, and safety needs. RN licensure is required.

Qualifications

• BSN required; MSN preferred.
• RN licensure is required.
• 3–5 years of relevant case management experience.
• Health care team coordination and communication skills.

Responsibilities

• Manages hospitalization caseload from pre-admission through one week post-discharge.
• Facilitates coordination of care with health care team to achieve outcomes.
• Completes educational responsibilities.
• Performs leadership duties.
• Performs quality assurance and performance improvement duties.
• Performs communication and networking duties.
• Performs systems-based practice duties.
• Addresses physical, psychosocial, educational, and safety needs of patients; all ages.

• Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge.

• Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.

• Current Health Care Provider BLS training from the American Heart Association is required.
