Qualifications

• Bachelor's degree required in a relevant field (e.g., Physical Therapy).
• Licensed Physical Therapist (PT) in good standing (IDFPR).
• Basic Life Support (BLS) certification within 30 days (AHA).

Responsibilities

• Performs patient transfers with safe body mechanics.
• Delivers compassionate care and engages patients, families and staff.
• Designs and adjusts a plan of care with goals, outcomes, and discharge planning.
• Educates patients and caregivers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Conducts examinations with reliable methods based on history and screening.
• Documents patient care in accordance with policy.
• Screens patients to determine need for therapy services.
• Supervises PTA and rehab technicians as appropriate.
• Communicates discharge recommendations and equipment needs to medical team.
