Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, modify plans of care as needed, and act as a clinical resource for staff and students. You will model professional service delivery and contribute to high-quality rehabilitation.

Required: licensed PT (IDFPR) and BLS within 30 days; ongoing education in acute stroke care; and ability to document and communicate effectively with the care team. A sign-on bonus is available.

Qualifications

• Licensed Physical Therapist (PT) – IDFPR
• Basic Life Support (BLS) within 30 days – American Heart Association
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients

Responsibilities

• Essential Functions: Safe patient transfers with proper body mechanics.
• Demonstrates respectful, empathetic care and engagement with patients and staff.
• Designs and adjusts care plans integrating goals, treatments, outcomes, and discharge planning.
• Educates patients, families, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations using reliable, valid methods.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine therapy needs and effectiveness.
• Supervises PTA and rehab technicians per regulatory guidelines.
• Communicates discharge disposition and equipment needs to the medical team.

Bachelor's Degree in Physical Therapy
