Job summary

Carle Health is seeking a Licensed Physical Therapist to evaluate and treat patients of all ages, modifying care plans as needed. Provides clinical leadership, education to staff and students, and acts as a quality resource within the rehabilitation team.

Responsibilities include developing discharge plans, documenting according to policy, and ensuring safe transfers and body mechanics. Sign-on bonus is offered where applicable, with opportunities across our health system.

Qualifications

• Licensed Physical Therapist (PT) – IDFPR
• Basic Life Support (BLS) within 30 days – American Heart Association (AHA)
• Complete required continuing education for acute stroke care

Responsibilities

• Transfers patients safely with assistance as needed and uses proper body mechanics
• Treats patients and staff with dignity and respect while staying engaged in care
• Designs and adjusts a plan of care with goals, outcomes, and discharge plans
• Educates patients and families using effective teaching methods
• Evaluates findings to determine PT diagnoses and outcomes
• Performs examinations with reliable methods
• Documents in accordance with CTSCP67
• Screens patients for therapy needs and uses support personnel appropriately
• Complies with DNV Hip and Knee Replacement Program requirements (if applicable)
• Evaluates and implements functional intervention plans
• Uses standardized outcomes measures to reflect functional performance
• Completes documentation to regulatory standards
• Collaborates with patient/family and medical team
• Provides supervision to PTA and rehabilitation technicians
• Communicates discharge recommendations and equipment needs to the medical team
