Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages, adjusting plans of care as needed. You will educate patients and families, document outcomes, and collaborate with the medical team to ensure safe, effective discharge planning.

Qualifications

Bachelor's Degree in Physical Therapy is required.
Licensed Physical Therapist (PT) must be licensed by the Illinois Department of Financial and Professional Regulation (IDFPR).
Basic Life Support (BLS) certification within 30 days is required; continuing education for acute stroke care is expected.

Responsibilities

Designs and implements a plan of care, adjusts based on patient response and evidence.
Educates patients, families, and caregivers using effective teaching methods.
Performs examinations and documents findings per CTSCP67; ensures accurate patient records.
Supervises PTA and rehab technicians; communicates discharge plans and equipment needs.
