Qualifications

• Professional degree in physical therapy and current licensure is required.
• Active BLS certification within 30 days of hire is expected.
• Continued education to meet Stroke Care requirements may be required.

Responsibilities

• Designs the plan of care integrating goals, treatment, outcomes, and discharge plan.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations using reliable methods relevant to the chief complaint and history.
• Educates patients, families and staff using effective teaching methods.
• Monitors care delivered by support personnel and provides regular feedback.
• Documents in accordance with CTSCP67 and regulatory standards.
• Collaborates with the rehab team and communicates discharge needs and equipment.
