Job summary

The Cancer & Hematology Centers is seeking a New Patient Referral Specialist to support and collaborate with the clinic team in delivering excellent patient care. This role focuses on intake coordination, communication with referring providers, and managing schedules for new patients.

Qualifications include a high school diploma or equivalent, completion of orientation, and at least one year in outpatient healthcare.

Qualifications

• Required High School Diploma or equivalent.
• Required to successfully complete orientation program.
• Preferred one year experience in an outpatient physician practice or healthcare setting.

Responsibilities

• Act as a liaison between CHC and referring provider offices/hospital systems to effectively manage the intake and scheduling process of all new patients.
• Maintain clear communication with referring entities throughout the intake process, ensuring timely updates are provided.
• Manage provider schedules to facilitate scheduling of new patients with appropriate sub-specialty providers.
• Maintain frequent communication with providers and leaders about schedule changes and new patient details.
• Finalize patient entry by verifying demographics and insurance information, creating a new chart, and obtaining medical records.
• Complete insurance verification and obtain global authorization when needed, addressing insurance gaps to ensure early intervention.
• Support patient portal enrollment to assist value-based care measures and patient access to information.
• Coordinate delivery and execution of pre-visit forms.
• Maintain a dynamic wait list for sooner visit opportunities and assist with quality improvement.
• Engage in continuous education to maintain skills and competency.
