Job summary

The Arboretum of Winnie in Katy, TX is seeking a licensed Physical Therapist to evaluate and treat patients, design individualized rehabilitation plans, and operate within an interdisciplinary team. You will deliver prescribed therapies and monitor progress according to the plan of care.

In this role you will document meticulously, collaborate with staff, and educate families. Texas licensure and ongoing compliance are essential, with flexible schedules and supportive rehab leadership.

Qualifications

• Active Texas license in Physical Therapy is required.
• Experience in SNF/long-term care is preferred.
• Knowledge of Net Health EDS is a plus.

Responsibilities

• Consult with patients to learn about their physical condition.
• Assess evaluations and test results to guide care.
• Develop and implement personalized rehabilitation plans.
• Administer prescribed physical therapy treatments.
• Monitor patient progress and adjust plans as needed with IDT input.
• Provide education to patients and families.
• Collaborate with the interdisciplinary team to ensure quality care.
• Ensure timely and compliant documentation.
