Job summary

Qualifications

Responsibilities

• Directs safe and effective delivery of chronic hemodialysis therapy.
• Educates patients about ESRD, treatment, and adherence.
• Documents patient care and lab results in Medical Information System.
• Coordinates with physicians and healthcare team for care plans.
• Maintains facility safety and regulatory compliance.
• Supports staff training and participates in CQI activities.
