Job summary

Evolve Therapy Services in partnership with a Skilled Nursing Facility in Wabasso, MN is seeking a licensed Physical Therapist for an in-house role. This full-time position includes a $5,000 sign-on bonus and tuition reimbursement, with part-time or PRN options also available.

You will evaluate residents, develop treatment plans, document care in NetHealth EMR, and collaborate with an interdisciplinary team to deliver high-quality therapy services.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to documentation and billing procedures.
• Develop short- and long-term treatment plans with measurable goals.
• Evaluate residents to obtain data for treatment planning.
• Communicate findings and recommendations to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
