Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility located in Franklin, MN. The position offers full-time, part-time, or PRN options, with benefits such as tuition reimbursement and competitive pay.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill therapy services according to procedures.
• Evaluate residents and develop treatment plans with short and long-term goals.
• Conduct screenings to determine need for intervention and plan follow-up actions.
• Communicate evaluation findings and treatment plans to residents, families, and interdisciplinary team.
