Job summary

Norman Fixel Institute for Neurological Diseases is seeking a Clinical Case Manager I to provide psychosocial casework for patients and families dealing with complex neurological conditions. The role supports navigation across care settings and coordinates with multiple teams to improve patient outcomes.

The position requires licensure as a social worker in Florida and offers full-time hours at the UF FIND department with opportunities for professional development and program leadership

Qualifications

• Master's degree in an appropriate area; or a bachelor's degree in an appropriate area and two years of relevant experience.
• Florida social worker licensure required.
• Experience in psychosocial assessment and care coordination.
• Ability to work with interdisciplinary teams.

Responsibilities

• Develop and implement individualized treatment plans aligned with patient needs.
• Provide patient navigation across care settings (inpatient, outpatient, home health, etc.).
• Deliver education and support programming for patients and families.
• Coordinate outreach with community resources, agencies, and advocacy groups.
• Track and report on program initiatives and outcomes.
• Maintain and log patient data in tracking systems.
