Job summary

Cleveland Clinic Health System in Cleveland is expanding its Neurological Institute with a new state-of-the-art facility and seeks a Case Management Social Worker to join the neuro team embedded on the nursing units.

You will coordinate transitions, provide psychosocial assessments, discharge planning, and advocacy to diverse patients and families, with an 8:00am–4:30pm schedule and onboarding in 8–12 weeks.

Qualifications

• Master's Degree in Social Work or MSSA.
• Current state licensure as a Social Worker (LSW/LISW/LISW-S).
• BLS certification through AHA or Red Cross.

Responsibilities

• Coordinate transitions across the continuum of care to ensure timely discharges.
• Communicate discharge planning status with interdisciplinary teams.
• Educate patients and families on post-acute options and finances.
• Conduct psychosocial assessments and interventions to engage patients/families.
• Advocate for patients and families and provide end-of-life counseling when needed.
• Support compliance with regulatory requirements and ongoing education.
