Job summary

The ideal candidate holds a Bachelor's degree, a Florida RN license (or equivalent) with 5+ years of case management experience; CCM certification is preferred.

Qualifications

• Education: Bachelor’s Degree.
• Preferred Diploma/Associates Degree in Nursing and Professional Certification in Case Management.
• Licensure/Certification/Registration: Florida Registered Nurse.
• License Skills: Effective communication with physicians, patients/families, and staff; minimum 5 years case management experience; CCM certification preferred; English proficiency required.

Responsibilities

• Assess patient and family scenarios for clinical appropriateness and plan management of the continuum of care.
• Perform assessment, planning, intervention and reassessment in a collaborative environment.
• Establish proactive, individualized plans to optimize quality of care, utilization, cost and patient satisfaction.
