Job summary

Pacific Clinics in California is seeking a case manager to support neurodivergent clients and their families under supervision. You will coordinate community-based supports, crisis stabilization, and placement in the least restrictive setting while promoting the client’s dignity and self-determination.

The role requires a bachelor’s degree in psychology, social work, or related field and bilingual Spanish proficiency.

Qualifications

• Bachelor's degree in Psychology, Social Work, Social Welfare, Education or related field and one (1) year of experience required, or equivalent combination of education and experience (two years of relevant experience equals one year of education).
• Bilingual in Spanish required.

Responsibilities

• Collaborates with neurodivergent clients, caregivers and ancillary services providers to ensure long term community-based supports and access to services.
• Provides intensive short-term crisis supports and safety planning for neurodivergent clients and families.
• Meets with neurodivergent clients and caregivers in home and community settings to develop and sustain supports.
• Helps stabilize crises and maintain the least restrictive home placement.
• Builds family and community connections and facilitators visits and supports natural supports.
• Maintains communication with authorized representatives and team members, participating in client/family team meetings and updates.
