The role requires a Bachelor’s degree in healthcare or related field, with CRC/CCM certification preferred and up to 30% travel.

Qualifications

• Bachelor’s degree in healthcare, human services, or a related field is required; Master’s degree is preferred.
• CRC or CCM certification is preferred.
• Hands-on case management experience in healthcare.
• Ability to travel up to 30% and lift up to 50 lbs.

Responsibilities

• Lead interdisciplinary treatment team meetings to set, monitor, and revise rehabilitation goals.
• Coordinate therapy schedules and maintain clinical documentation.
• Advocate for client needs and provide guidance across all phases of care.
• Collaborate with Clinical Director, direct care staff, and physicians to support care plans.
• Maintain consistent contact with families and stakeholders with updates and education.
• Monitor funding guidelines and complete required documentation for billing integrity.
• Maintain thorough, timely client charts in compliance with HIPAA/HITECH and state regulations.
• Assist with clinical data collection for quality assurance.
