Job summary

PeaceHealth Rehab & Wellness Center in Florence, Oregon is seeking a Physical Therapist - Outpatient Neuro for a Full Time, 0.90 FTE, Day position. The salary range is $51.55 - $72.78 per hour, with hiring rate dependent on education, experience, and applicable agreements.

The successful candidate will evaluate neuro patients, establish a plan of care, document thoroughly, and collaborate with an interdisciplinary team to provide high quality care. Relocation and signing bonus may be available.

Qualifications

• Evaluation of patients and goal setting for PT care.
• Licensed Physical Therapist in Oregon (OCPE).
• Basic Life Support required upon hire.

Responsibilities

• Evaluates assigned patients and establishes a treatment plan and goals.
• Provides patient treatment according to plan of care and updates it as needed.
• Completes documentation accurately and timely to regulatory standards.
• Collaborates with other healthcare professionals to ensure continuous care.
• Mentors and oversees new staff and students as appropriate.
• Contributes to development of new services and quality improvement activities.
