PeaceHealth Home Health in Florence, Oregon seeks a Physical Therapist for a Full Time, 1.00 FTE, Day position. The compensation includes a salary range of $51.55– $72.78 per hour, with relocation assistance possible and a hiring bonus.

The role involves evaluating patients in their homes, coordinating care with a multidisciplinary team, and providing education to patients and families. Ideal candidates hold a Physical Therapy degree and Oregon licensure, with at least one year of PT experience;

Qualifications

• License to practice Physical Therapy in Oregon
• BLS/CPR required on hire
• Foreign degree equivalency accepted if applicable

Responsabilités

• Evaluate patients and establish a treatment plan and goals
• Provide treatment per plan and adjust as needed
• Complete documentation timely and accurately per regulations
• Collaborate with healthcare professionals to ensure continuous patient care
• Mentor and supervise students or assistants as needed
• Contribute to development of new services and quality improvement
• Participate in meetings and training sessions as required
• Recommend content and scope of therapy services and implement policies
