Job summary

Rehab Without Walls is seeking an Occupational Therapist specializing in neuro rehab for home and community settings in New Hampshire. You will design and implement individualized treatment plans for adults with stroke, TBI, SCI, and Parkinson’s, delivering true one-on-one care in patients’ own environments.

You’ll have flexible scheduling, autonomy over caseloads, and local coverage with paid documentation time and mileage reimbursement.

Qualifications

• Bachelor’s degree in Occupational Therapy from an accredited program.
• Current, unrestricted OT license in the practicing state.
• Current CPR certification.
• Minimum one year of work experience as an Occupational Therapist.
• Knowledge of rehabilitation techniques related to complex neurological diagnoses preferred.

Responsibilities

• Perform evaluations and develop treatment plans
• Regularly reassess effectiveness of treatment plans, attend staffing meetings and family conferences
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
