Job summary

Rehab Without Walls Neuro Rehabilitation in New Hampshire is seeking a Physical Therapist (PT) specializing in neuro home and community care. You will provide one-on-one, in-home neuro rehabilitation with flexible scheduling and autonomy over your caseload.

Join a team focused on meaningful functional outcomes for patients recovering from neurological conditions such as stroke, TBI, and Parkinson’s disease, delivering care in patients’ homes and community settings with local coverage.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted license as Physical Therapist in the practicing state.
• Current BLS/CPR certification compliant with state regulations.
• Minimum of one year of work experience as a physical therapist preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members.
• Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.
