Job summary

Mass General Brigham is seeking dedicated Case Managers to join the Case Management department at 265 Charles Street, Boston, MA. The RN Case Manager collaborates across teams to ensure efficient patient care, coordinate discharge planning, and optimize the care trajectory for diverse patients.

The role is onsite with 5–8 hour shifts and a weekend commitment. Minimum 2 years Med/Surg experience required; ADN required, BSN preferred; Case Manager certification preferred.

Qualifications

• RN Case Manager experience required.
• Licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborates to enhance patient care delivery across the continuum.
• Evaluates and facilitates the trajectory of patient care.
• Coordinates care with the healthcare team to meet patient needs.
• Supports onboarding and professional development programs for staff.
