Qualifications

• Master’s degree in social work (MSW) or Master of Science in Social Administration (MSSA).
• Current state licensure as a Social Worker (LSW, LISW or LISW-S).
• Basic Life Support (BLS) certification.
• Valid Ohio Driver’s license upon hire.

Responsibilities

• Coordinate the care and services of select patients.
• Intervene with patients who have complex psychosocial and discharge planning needs.
• Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling.
• Offer crisis interventions to patients and families and facilitate the transition of care for high‑risk patients facing barriers to care.
• Manage all aspects of discharge planning for assigned patients to ensure timely discharge and post‑discharge connections.
